Information Change — Name

Follow this procedure to inform the Certification Department that you have changed your name.

  1. Print/obtain a copy of the current Certification Form 2.

  2. Page 1 – Complete “PERSONAL INFORMATION” section only.

  3. Page 2 – Check the box for “Change of Name”.

  4. Sign and date the form!

  5. Submit your form to the OSSTF/FEESO Certification Department at the address or fax number at the top of Form 2.
