Certification Application Forms and Procedures

In this section, you will find information on forms, procedures and appeals.

Please be aware that the Certification Application Forms are currently not available for submission on-line. Please download the appropriate PDF Application Form and either fax to 416-751-0910 or mail directly to the Certification Department at OSSTF/FEESO 60 Mobile Drive Toronto, ON M4A 2P3. We apologize for any inconvenience.

There are four Certification Forms to be used in specific circumstances. Read the Procedures section further down on this page for detailed instructions on filling out the forms.

Procedures

Visit the links below for more information on the specific procedure to follow to complete a variety of Certification tasks.

Use Certification Form 1 for

1. Initial Certification Rating Statement (CRS)

  • You are a dues-paying member or a voluntary member and have not been evaluated by the OSSTF/FEESO Certification Department.

2. Changing a Letter of Appraisal to an initial Certification Rating Statement

  • You are a new member with an existing evaluation from OSSTF/FEESO

3. Changing a Letter of Assessment to an initial Certification Rating Statement

  • You are a new member with an existing evaluation from OSSTF/FEESO received as a Faculty of Education student

Use Certification Form 2 for

1. Re-evaluation for a new Certification Rating Statement

  • You have completed course(s) or degree(s) or believe recent changes to the Certification Plan could mean a change in Group

2. Up-grading information

  • You would like direction on how to move to a higher Group

3. Changing a provisional Certification Rating Statement to a permanent Certification Rating Statement

  • You have received the documentation that was pending and now can forward it to the Certification Department

4. Changing a Letter of Evaluation to a Certification Rating Statement

  • Effective September 1, 2006 any teacher with a Letter of Evaluation can apply for a permanent CRS from OSSTF/FEESO

5. Duplicate Certification Rating Statement

  • You have need of another copy of the CRS already provided by OSSTF/FEESO

6. Name change

  • The name on your file in the Certification Department has changed

7. Address change

  • The address on your file in the Certification Department has changed

Use Certification Form 3 for

Course Approvals

  • You wish to take a course for up-grading and would like to have a guarantee that it will help with a move to a higher Group

Use Certification Form 4 for

Letter of Appraisal

  • You are not a member of OSSTF/FEESO but would like to know what your rating would be under the OSSTF/FEESO plan ($125.00 cost).
