OSSTF/FEESO Provincial Standing Committees and Councils Application Form

2012 - 2013

A separate application form must be completed for each committee/council.

Complete written, faxed and electronic applications, including the Confidential Report of the Provincial Councillor, must be received by Lisa Rogers, Administrative Assistant to Provincial Council by March 1, 2012.

Any application received after March 1, 2012 but before May 1, 2012 will be considered a late application, and will only be considered for appointment if there are insufficient on-time applications on file at Provincial Office.

For your records, a copy of your submitted form will be sent to the e-mail address that you provide in your application.
